RESUMEN Introducción: Los trabajadores de la salud se enfrentan permanentemente a labores complejas que afectan la región lumbar y miembros inferiores. Objetivo: Identificar la prevalencia de enfermedad osteomioarticular lumbosacras y miembros inferiores en auxiliares de enfermería Método: Se realizó un estudio epidemiológico, descriptivo y transversal en una muestra de 46 auxiliares del Distrito 18D04 de salud de Tungurahua, Ecuador. El instrumento aplicado fue un cuestionario para indagar variables sociodemográficas, antigüedad en el sector, horarios, tipos y tiempo de exposición a las labores habituales. Se utilizó la prueba estadística Chi-cuadrado, la paramétrica de correlación de Pearson y la prueba t de Student. Resultados: El 85 % de los estudiados presentó enfermedades lumbosacras y miembros inferiores durante el tiempo de trabajo, se apreció que el 39 % ha tenido una baja temporal al trabajo entre 1-3 días. Se identificó que la carga de peso es realizada por un 37 % y caminatas prolongadas en un 33 %. El 67 % tenía entre 11 y 40 años de trabajo y el resto menos de 10 años en el sector. El 54 % padeció enfermedades pertenecientes a miembros inferiores y un 46 % a lumbosacras. Conclusiones: El padecimiento de enfermedad osteomioarticular lumbosacras y miembros inferiores está relacionado con la edad y con padecerlas en los últimos doce meses, estas han provocado ausentismo laboral. No se comprobó una relación estadísticamente significativa con el padecimiento de enfermedad osteomioarticular con la antigüedad en el sector, las jornadas de trabajo y las actividades que más desarrollan.


ABSTRACT Introduction: Health care workers are constantly confronted with complex tasks that affect the lumbar region and lower limbs. Objective: To identify the prevalence of lumbosacral and lower limb osteomyoarticular pathologies in nursing assistants. Method: An epidemiological, descriptive and cross-sectional study was carried out in the 18D04 health district of Tungurahua, Ecuador. A total of 46 nursing assistants were selected in this study. We aimed a questionnaire to assess the sociodemographic variables, seniority in the health sector, time schedules, modes and time of exposure to the usual work. Chi-square test, Pearson's parametric correlation test, and the Student's t-test were used. Results: Most nursing assistants, the 85%, were diagnosed with lumbosacral and lower limb pathologies, and 39% took a temporary absence from work between 1-3 days. It was found that 37 % performed the heavy load work and 33% performed prolonged walks. The 67% had between 11 and 40 years of work and the rest had less than 10 years in the health sector. Fifty-four percent suffered from lower limb pathologies and 46% from lumbosacral pathologies. Conclusions: Lumbosacral and lower limb osteomyoarticular pathology is related to age, and their suffering in the last 12 months has led to a level of absenteeism in the workplace. There was no an important relationship, statistically, between osteomyoarticular pathology and seniority in the sector, working hours and the activities most frequently performed.


RESUMO Introdução: Os trabalhadores da saúde enfrentam constantemente tarefas complexas que afetam a região lombar e membros inferiores. Objetivo: Identificar a prevalência de doenças osteomioarticulares lombossacrais e de membros inferiores em auxiliares de enfermagem. Método: Estudo epidemiológico, descritivo e transversal em uma amostra de 46 auxiliares do Distrito Sanitário 18D04 de Tungurahua, Equador. O instrumento aplicado foi um questionário para investigar variáveis sociodemográficas, antiguidade no setor, horário, tipos e tempo de exposição a tarefas normais. Foram utilizados o teste estatístico Qui-quadrado, correlação paramétrica de Pearson e teste t de Student. Resultados: 85% dos estudados apresentaram doenças lombossacrais e de membros inferiores durante o tempo de trabalho, observou-se que 39% tiveram afastamento temporário do trabalho entre 1-3 dias. Identificou-se que a descarga de peso é realizada por 37% e as caminhadas prolongadas por 33%. 67% tinham entre 11 e 40 anos de trabalho e o restante menos de 10 anos no setor. 54% sofreram doenças pertencentes aos membros inferiores e 46% lombossacral. Conclusões: O sofrimento da doença osteomioarticular lombossacral e de membros inferiores está relacionado à idade e com o sofrimento nos últimos doze meses, estes têm causado absenteísmo ao trabalho. Não foi verificada relação estatisticamente significativa entre o sofrimento de doença osteomioarticular e o tempo de permanência no setor, a jornada de trabalho e as atividades que mais desenvolvem.

Summary Objective: To analyze factors associated with chronic joint symptoms (CJS) in adults. Method: A population-based, cross-sectional study was performed with a sample of 1,217 adults aged between 20 and 59 years, in the city of Viçosa, in 2014. The sampling process was performed by conglomerates and sample was selected using a two-stage cluster-sampling scheme. First, 30 of the 99 census tracts of Viçosa were randomly selected using a random sampling scheme, without replacement. Household questionnaires were applied to obtain CJS data, sociodemographic conditions, behavioral factors and health status. Multivariable analysis was conducted using Poisson regression, adjusted for the sampling design effect, using the svy commands in Stata software. Results: Prevalence of CJS totaled 31.27%, significantly higher in women (18.45). Age ranges 40-49 (PR 1.50; 95CI 1.16-1.92) and 50-59 years (PR 1.55; 95CI 1.07-2.25); overweight (PR 1.60; 95CI 1.28-2.00); obesity (PR 1.60; 95CI 1.11-2.29); and those who self-reported performing heavy work (PR 1.27; 95CI 1.09-1.48) showed higher prevalences of CJS. Conclusion: Women and individuals who were older, overweight and performing heavy work had a higher risk of CJS in this adult population residing in Viçosa, MG, Brazil.


Resumo Objetivo: Analisar os fatores associados aos sintomas articulares crônicos (SAC) em adultos. Método: Trata-se de um estudo transversal, de base populacional, com 1.217 adultos, na faixa etária de 20 a 59 anos, na cidade de Viçosa, MG, 2014. A amostragem foi realizada por conglomerados em duplo estágio, sendo as unidades de primeiro estágio os setores censitários, e, em seguida, os domicílios. Foram sorteados 30 setores dentre os 99 de Viçosa, por meio de amostragem casual simples, sem reposição. A coleta de dados foi composta por aplicação de questionário contendo questões relativas a variáveis de SAC, sociodemográficas, comportamentais e de saúde. Para verificar as associações, apresentaram-se proporções, razões de prevalência e seus respectivos intervalos de confiança de 95%. A análise múltipla foi realizada por meio da regressão de Poisson, utilizando o conjunto de comandos svy do software Stata, o qual considera o efeito da expansão da amostra na análise dos dados. Resultados: A prevalência estimada de SAC foi de 31,27%, maior nas mulheres (18,45%). Estiveram associadas ao SAC as mulheres (RP 1,49; IC95% 1,23-1,81); as idades de 40 e 49 (RP 1,50; IC95% 1,16-1,92) e 50 e 59 anos (RP 1,55; IC95% 1,07-2,25); o sobrepeso (RP 1,60; IC95% 1,28-2,00); a obesidade (RP 1,60; IC95% 1,11-2,29); e aqueles que autorreferiram realização de trabalho pesado (RP 1,27; IC95% 1,09-1,48). Conclusão: O sexo feminino, a faixa etária de 40 a 59 anos, o sobrepeso, a obesidade e a realização de trabalho pesado foram fatores de risco para SAC em adultos de Viçosa, MG.

ABSTRACT

Objetivo: Avaliar a associação entre urticária crônica e doenças sistêmicas (síndrome metabólica, artropatias, doença vascular periférica e do trato gastrintestinal). Métodos: Estudo transversal, descritivo, de base clínica. Foi realizado um levantamento de dados por meio de prontuários de 95 pacientes, que estiveram em tratamento ambulatorial de rotina de abril de 2014 a abril de 2015, mediante assinatura de um Termo de Consentimento Livre e Esclarecido pelo paciente durante suas consultas de rotina no Ambulatório de Urticária. Para analisar a associação das doenças, foi utilizado o teste estatístico qui quadrado, considerando sexo feminino e masculino, e a doença. Consideraram- se relevância estatística as relações com p abaixo de 0,05 (p<0,05). O nível de significância adotado foi de 95%. O programa utilizado foi o Stata 11.0. Resultados: Artropatias foram positivas em aproximadamente 55% das mulheres do estudo e 17% dos homens (p=0,001 e p<0,05, respectivamente). Houve significância estatística na relação entre doença vascular e urticária crônica (p=0,022 e p<0,05, respectivamente). Conclusão: Não foi encontrada associação relevante estatisticamente para afirmar que houve confiança na associação entre diabetes e urticária crônica (p=0,801 e p>0,05, respectivamente). Não se obteve relevância na relação com doenças do trato gastrointestinal (p=0,437/p>0,05). relevância na relação com doenças do trato gastrointestinal (p=0,437/p>0,05).


Objective: To evaluate the association between Chronic urticaria and systemic diseases such as metabolic syndrome, arthropathies, peripheral vascular disease, and gastrointestinal diseases. Methods: a cross-sectional, descriptive study of clinical basis. A survey of data was performed through the medical records of 95 patients who underwent routine outpatient treatment from April 2014 to April 2015, and signed na Informed Consent during their routine appointments in the Urticaria Clinic. To analyze the diseases associations we used the statistical Chi-square test, considering male and female patients, and the disease. Relations with p lower than 0.05 (p<0.05) were considered statistically relevant. Significance level was 95%. The software used was Stata 11.0. Results: Arthropathies were positive in approximately 55% of women in the study, and 17% of men (p=0.001 and p<0.05, respectively).There was a statistically significant relationship between vascular disease and chronic urticaria (p=0.022 and p<0.05, respectively). Conclusion: No statistically significant association was found to state that there was confidence in the association between diabetes and Chronic urticaria (p=0.801 and p>0.05, respectively). No relevant relation was observed with gastrointestinal diseases (p=0.437/ p>0.05).

BACKGROUND: The prevalence of os acromiale has been documented to be between 1% and 15% and is known to be clinically associated with subacromial impingement or rotator cuff tear. However, the prevalence of os acromiale in Korea has not yet been determined. The purpose of this study is to evaluate the prevalence of os acromiale in Korean patients who visited shoulder clinics and to investigate the correlations with rotator cuff tear. METHODS: We retrospectively reviewed the X-rays of patients visiting a shoulder clinic at a tertiary hospital in Korea from January 2011 to January 2012 to determine the frequency of os acromiale. X-ray findings were confirmed with magnetic resonance imaging (MRI) for patients who had these images available. MRI was also used to assess the status of the rotator cuff. The correlation between the presence of os acromiale either with gender, hand dominance or rotator cuff tear was analyzed statistically. RESULTS: A total of 2,946 shoulders from 1,568 patients were analyzed with X-rays. Thirteen cases out of 1,568 patients had an os acromiale; and there were five and eight cases of pre-acromiale and meso-acromiale, respectively. Thus, the prevalence of os acromiale in this study population was found to be 0.7 (7 cases per 1,000 patients). Bilaterality was found in two cases. Os acromiale was not more frequent according to gender (five males versus eight females, p = 0.525) and hand dominance was not associated with frequency of os acromiale (seven dominant arms versus six non-dominant arms, p = 0.631). A sub-analysis of shoulders with available MRIs (1,074 shoulders) revealed that there were two rotator cuff tears (40%) out of five cases of os acromiale, whereas 607 rotator cuff tears were observed (57%) among 1069 cases without os acromiale. This difference was not statistically significant (p = 0.656). CONCLUSIONS: The identified prevalence of os acromiale in Korean patients who visited shoulder clinics is 0.7%, which is much lower as compared with the prevalence of general population from other ethnic groups. No correlation was observed between rotator cuff tears and os acromiale in this study population.

OBJECTIVE: To study the prevalence of Jaccoud arthropathy (JA) in a sample of local systemic lupus erythematosus (SLE) patients and its clinical and serological associations. METHODS: 308 SLE patients from a single university center for the last two years were interviewed and examined. The presence of JA was searched for according to the JA index. After this, charts were reviewed for clinical and serological profile. RESULTS: The studied sample was composed by 94.5% females and 5.5% males with mean age of 38.08 ± 12.04 years and mean disease duration of 29.68 ± 11.63 years. A JA prevalence of 6.1% was found in this sample. There was a positive association of JA presence with arthritis complaints (p = 0.001) and a negative association with renal involvement (p = 0.028). Patients with JA had higher positivity for anti-dsDNA (p = 0.022). CONCLUSION: Despite the positive association of JA with arthritis and anti-dsDNA, there was a negative association with nephritis. This could suggest that JA patients belong with a SLE subset with a better prognosis.

ABSTRACT

A cross-sectional population-based study was conducted to identify the prevalence of common mental disorders and verify the association with chronic non-communicable diseases (NCDs) and the self-reported number of chronic diseases. The Self-Reporting Questionnaire (SRQ-20) was applied in a multi-stage random sample of 1,276 adults aged 40 and older. Socio-demographic, behavioral, and health-related variables were also obtained using a structured questionnaire. Prevalence of common mental disorders was 30.2 percent. Lower schooling and social class and the 46-55-year age bracket were associated with psychiatric morbidity. Each chronic illness was independently associated with common mental disorders. However, a stronger association was found between common mental disorders and the total number of self-reported chronic conditions, with a prevalence ratio of 4.67 (95 percentCI: 3.19-6.83) for five or more self-reported NCDs. The current study emphasizes the importance of common mental disorders in chronically ill patients, particularly in those with more total chronic conditions.


Estudo transversal de base populacional foi conduzido tendo como objetivos avaliar a prevalência dos transtornos mentais comuns e verificar sua associação com determinadas enfermidades crônicas e com o número de doenças crônicas relatadas pelo indivíduo. Para a avaliação de transtornos mentais comuns, o Self-Reporting Questionnaire (SRQ-20) foi aplicado em 1.276 adultos com 40 anos ou mais. Variáveis sócio-demográficas, comportamentais e relacionadas à saúde foram obtidas por meio de um questionário estruturado. Os transtornos mentais comuns apresentaram uma prevalência de 30,2 por cento, estando associados à baixa escolaridade e classe social, e à faixa etária de 46-65 anos. Todas as doenças crônicas pesquisadas mostraram-se associadas aos transtornos mentais comuns. Entretanto, o número de enfermidades apresentadas pelo indivíduo teve maior importância do que cada uma delas individualmente, com uma razão de prevalência de 4,67 e intervalo de 95 por cento de confiança: 3,19-6,83 para cinco ou mais doenças relatadas. O presente estudo realça a importância de se atentar para os transtornos mentais em indivíduos com enfermidades crônicas, principalmente naqueles que se apresentam com um grande número de doenças.

ABSTRACT

Empregou-se a estimulação elétrica neuromuscular (EENM) de baixa freqüência no músculo quadríceps femoral de cães com atrofia induzida e avaliou-se a ocorrência de ganho de massa nessa musculatura. Foram utilizados oito cães com pesos entre 15 e 30kg, distribuídos aleatoriamente em dois grupos denominados de I ou controle e II ou tratado. A articulação femorotibiopatelar esquerda foi imobilizada por 30 dias pelo método de transfixação percutânea tipo II, com retirada de aparelho de imobilização após esse período. Decorridas 48 horas da remoção, foi realizada a EENM nos cães do grupo II, cinco vezes por semana, com intervalo de 24 horas cada sessão, pelo período de 60 dias. Foram avaliadas a circunferência da coxa, a goniometria do joelho, a análise clínica da marcha, as enzimas creatina-quinase (CK) e aspartato-amino-transferase (AST) e a morfometria das fibras musculares em cortes transversais do músculo vasto lateral colhido mediante biópsia muscular. A EENM foi empregada no músculo quadríceps femoral na freqüência de 50Hz, duração de pulso de 300 milisegundos e relação de tempo on/off de 1:2. Quanto à morfometria das fibras do músculo vasto lateral, no grupo tratado houve aumento significativo (P<0,05) da área transversal aos 90 dias em relação ao dia zero. A EENM de baixa freqüência ocasiona hipertrofia do músculo vasto lateral em cães após a imobilização rígida temporária da articulação do joelho.


Low frequency neuromuscular electrical stimulation (NMES) was used on the femoral quadriceps of dogs with induced muscular atrophy and the occurrence of gain in mass in these muscles was evaluated. Eight dogs from 15 to 30kg were randomly distributed in two groups named I, or control; and II, or treated. For the induction of muscular atrophy, the left femoral-tibial-patellar joint was immobilized for 30 days by percutaneous transfixation type II. After 30 days, the immobilization device was removed. The NMES treatment began 48 hours after the removal of the immobilization device of the dogs of group II, and it was carried out five times per week with an interval of 24 hours between each session, for 60 days. The following parameters were measured: thigh circumference, goniometry of the knee, clinical gait analysis, creatine kinase (CK) and aspartate aminotransferase (AST) enzymes, and morphometry of the muscular fibers in transversal cuts of the vastus lateralis muscle collected through muscular biopsy. The NMES was applied on the femoral quadriceps at a frequency of 50 Hz, with pulse duration of 300 milliseconds, and the on/off time was at a proportion of 1:2. Regarding the morphometry of the vastus lateralis fibers, a significant increase (P<0.05) in the transversal area of the treated group at 90 days was observed when compared with that identified at the time of immobilization. Thus, it can be concluded that low frequency NMES brings about hypertrophy of the vastus lateralis muscle in dogs after temporary rigid immobilization of the knee joint.

ABSTRACT

Objetivo. Hochberg menciona la alta prevalencia de espondilitis anquilosante en los pimas. Se decidió buscar espondiloartropatías (Esp A) antes del mestizaje o en los inicios del mismo. Material y Métodos. Se revisaron todos los documentos y descripciones de los pimas al contacto español, en la colonia y después de la Independencia. Resultados. Las descripciones más importantes fueron seis, de 1708 a 1795, en contacto con los pimas de seis a treinta años y ninguno describe datos de EA ni incapacidades. Discusión. La veracidad de los documentos puede cuestionarse pero al único médico no describe ninguna cura local para problemas que sugieran Esp A y otras descripciones hablan de la gran salud de éstos indígenas. La ausencia al trabajo era severamente castigada por lo que las incapacidades hubieran sido evidentes. El presente trabajo sugiere la ausencia de Esp A en los indígenas pimas en la época colonial y abre el camino para estudios antropológicos e inmunogenéticos

ABSTRACT

Objetivo. Las enfermedades articulares están entre las causas más frecuentes de invalidez que afectan a la población adulta. El propósito de ésta investigación fue determinar la prevalencia de manifestaciones articulares y las características demográficas de quienes las padecen, en el área metropolitana de Monterrey, usando visitas domiciliarias y un cuestionario sobre características socioeconómicas y clínicas. Métodos. Las personas con síntomas articulares fueron evaluadas por el Servicio de Reumatología. De 189 familias encuestadas, 19.5 por ciento tuvieron al menos un miembro con datos de enfermedad articular. Resultados. En total se encontraron 40 personas afectadas: 80 por ciento con afección poliarticular, 20 por ciento oligoarticular y 95 por ciento de los casos eran crónicos. En relación con la participación extrarticular, se encontró dolor muscular en 65 por ciento y lumbalgia en 47 por ciento de las personas detectadas. Los síntomas y signos articulares fueron más frecuentes en mujeres con una relación de 4:1, y el grupo de edad en quienes se encontró más frecuentemente fue en la sexta década de la vida, con rango de 17 a 90 años. El 22.5 por ciento de pacientes nunca habían recibido tratamiento, y cuando lo recibieron, los fármacos más frecuentemente administrados fueron los antiinflamatorios no esteroideos en 80 por ciento (salicilatos en 25 por ciento). Menos de la mitad tenía afiliación médica específica y casi en una cuarta parte no habían recibido tratamiento. Aproximadamente, a la mitad de los casos se les había diagnosticado artritis reumatoide sin fundamentos adecuados

Se presenta un estudio de distribución de frecuencia de enfermedades profesionales en Chile, según una serie de variables, como clasificación según rubro de actividades económicas, etc. Se pone especial atención a aquellas que por su frecuencia y permanente aumento en un período de 10 años, tienen algún grado de comparación de tasas observadas y esperadas se determina el subregistro que existe en Chile para esos años, en relación a la denuncias de enfermedades profesionales que partícularmente hacen los organismos estatales administradores de la ley 16.744. ABSTRACT

Health is a central theme for all the aged since affects their situation in all spheres of life. The objective of this study was to identify the health conditions of an islandwide sample of non institutionalized elderly respondents. This study is part of a 31 page questionnaire concerning the socialdermographic status of the Puerto Rican elderly. The questions pertaining to the specific topic of health conditions consist of ten iotems about background information and ten questions pertaining to health. Structured interviews were carried out from September 1983 to September 1985. The tests of significance were those of Kendall's Tau B and C and Cramer's V. The health conditions of greatest magnitude in this populaltion (arthritis, hypertension) are similar to those found in the elderly elsewhere. The oral illness condition was more frequent in the rural population of the island. Socio-economic considerations were found to be an important influence for the magnitude of health conditions
